Community Health Workers (CHWs), also called Community Health Representatives (CHRs), help AHCCCS members navigate health care, connect to community resources, and manage chronic conditions.

Who Can Get CHW Services

You may be eligible if you meet at least one of these:

You have a chronic condition (e.g., diabetes, hypertension, long-term health issues).

You are at risk for developing a chronic condition.

You have a barrier affecting your health, like a disability, social isolation, or language barrier.

Note: Your eligibility must be documented by a licensed provider in your medical record.

What CHWs Can Help With

CHWs provide support tailored to your needs, including:

Navigating the health care system and connecting you to medical or social services.

Health education, prevention strategies, and chronic disease management tips.

Assistance with social determinants of health, like transportation, housing, or access to care.

How to Access Services

Talk to your health care provider – A licensed provider (doctor, nurse practitioner, or physician assistant) must refer you for CHW services.

Referral documented – The provider documents your eligibility in your medical record.

Certified CHW assigned – A CHW certified by the Arizona Department of Health Services (ADHS) and employed by an AHCCCS-registered provider will begin working with you.

Important Notes

CHW services must be deemed medically necessary to be covered by AHCCCS.

Members in certain long-term care programs or specific tribal arrangements may not be eligible for CHW services, but may have other support options available.
