Guided Care Management (GCM) is a care coordination organization dedicated to helping individuals navigate complex health, behavioral, and social systems. We provide personalized case management services for Medicaid-eligible clients across Texas and Arizona — including pregnant women, children, and individuals with behavioral health needs — as well as self-pay care plans for those not covered by Medicaid.

What health plans does GCM work with?
GCM is contracted with several major Medicaid managed care plans in Texas and Arizona. In Texas, these include WellPoint (Elevance), UnitedHealthcare Community Plan, Parkland HEALTHfirst, Aetna Better Health of Texas, and Cook Children's Health Plan. GCM provides case management and care coordination for behavioral health needs — this includes connecting clients to therapists, psychiatrists, crisis services, and community programs. We do not provide direct therapy or psychiatric treatment, but we work closely alongside those providers as part of your care team.

A dedicated case manager helps coordinate appointments, connect to community services, monitor progress, and support overall wellness and stability.
Who qualifies for BH-TCM services?
To qualify for BH-TCM, individuals generally need to be Medicaid-eligible in Texas or Arizona and have a documented behavioral health diagnosis. Our team will conduct an assessment during your intake to confirm eligibility and determine the appropriate level of care.
What does a BH-TCM case manager actually do?

How do I qualify for the Children & Pregnant Women Program?
To qualify for the HHSC Children and Pregnant Women (CPW) program, you must:
  • Be a pregnant woman with a high-risk condition, or a child with a health condition or developmental risk
  • Be eligible for Medicaid in Texas or Arizona
  • Need services to access essential medical, social, educational, or other related services
  • Desire and consent to these services

About This Program

What Is AHCCCS and Why It Matters

AHCCCS — the Arizona Health Care Cost Containment System — is Arizona's Medicaid agency. It provides health coverage to eligible low-income residents, and through a unique arrangement with federally recognized tribes, it also funds a specialized set of programs designed specifically for American Indian and Alaska Native communities.

Unlike standard Medicaid, tribal health programs under AHCCCS operate with distinct eligibility criteria, care delivery structures, and provider networks rooted in tribal sovereignty and the federal trust responsibility to tribal nations.
