What health plans does GCM work with?
GCM is contracted with several major Medicaid managed care plans in Texas and Arizona. In Texas, these include WellPoint (Elevance), UnitedHealthcare Community Plan, Parkland HEALTHfirst, Aetna Better Health of Texas, and Cook Children's Health Plan. We also serve Arizona Medicaid (AHCCCS) members. If you are enrolled in a different plan, contact us and we will help determine your coverage options.

Do you provide mental health services directly?
GCM provides case management and care coordination for behavioral health needs this includes connecting clients to therapists, psychiatrists, crisis services, and community programs. We do not provide direct therapy or psychiatric treatment, but we work closely alongside those providers as part of your care team.

Behavioral Health Targeted Case Management (BH-TCM) is a Medicaid-covered service that provides structured, person-centered support for individuals with mental health diagnoses, psychiatric conditions, or substance use challenges. A dedicated case manager helps coordinate appointments, connect to community services, monitor progress, and support overall wellness and stability.
Who qualifies for BH-TCM services?
To qualify for BH-TCM, individuals generally need to be Medicaid-eligible in Texas or Arizona and have a documented behavioral health diagnosis. Our team will conduct an assessment during your intake to confirm eligibility and determine the appropriate level of care.
What does a BH-TCM case manager actually do?
Your BH-TCM case manager will:
  • Help you schedule and attend behavioral health appointments
  • Coordinate between your therapist, psychiatrist, and other providers
  • Connect you to crisis services, housing support, or other community programs

  • Need services to access essential medical, social, educational, or other related services

About This Program

What Is AHCCCS?

AHCCCS is Arizona’s Medicaid agency. Through a unique arrangement with federally recognized tribes, it funds specialized programs designed specifically for American Indian and Alaska Native communities.

Tribal health programs under AHCCCS operate with distinct eligibility criteria and care delivery structures rooted in tribal sovereignty. GCM serves members enrolled in these programs with compassionate, whole-person care coordination.

TRBHA

Tribal RBHAs manage and deliver behavioral health services for tribal communities under a model that preserves tribal sovereignty while delivering AHCCCS-funded mental health and substance use services.

Who Qualifies

Generally, you must be an enrolled member or descendant of a federally recognized tribe and meet AHCCCS income and residency requirements.
